How to re-issue SRPK Card in case of Lost or Damage ?

In case, user loses his/her SRPK Card then user should report the nearest Police Station immediately and file a FIR for the lost SRPK Card. User will have to generate a ticket for informing SRPK representative regarding the lost card. After following this procedure, SRPK will re-issue the SRPK Card to the user and send it to user’s registered address by India Post.
In case, user’s SRPK Card has been damaged or broken i.e. SRPK Card number and SRPK Card validity date are not legible then user will have to send his/her damaged SRPK card at SRPK Head Office address. Users will also have to send valid Courier or Shipment tracking ID to SRPK through Support Ticket Panel, after which SRPK will re-issue his/her new SRPK Card and send it to user’s registered address by India Post.
In both cases, User will have to pay Rs.150 (Application Fee) to re-issue the SRPK Card.
